Job Duties &
Responsibilities:
Attends medical rounds with physician/surgeon and other clincal care providersAssessment, planning and implementation process for a select group of patients in need of orthopedic servicesFor pre-admission CJR patients, conduct a comprehensive pre-admission assessment and education on a timely basis, follow up to assure needed services are in place, and contact providers as neededFor post-op/post-hospitalization patients, conduct ongoing contact and follow-up with post-acute care providers and other providers on a timely basis and notify providers of readmissions and significant clinical status changes as appropriateCompletes a comprehensive health assessment of the patient s physical, psychological, social and environmental, financial and functional status in collaboration with multidisciplinary team membersPerform patient assessment, symptom management, telephone triage and offer patient/family educationBe responsible for providing excellent and adaptive nursing care based on the individual patient needs using the nursing process, which includes assessment, planning, implementation, and evaluationInteracting with physiciansHandling budgetsManaging difficult casesRequirementsRequired for Onboarding BLSCore Competency Recommended Skills Clinical Works Finance Financial Management Multidisciplinary Approach Nursing Nursing Process Estimated Salary: $20 to $28 per hour based on qualifications.
